In September 2015, the Industrial Commission of Ohio released its new form for injured workers applying for permanent total disability. Although the form is shorter than the previous form, there are additional sections requesting information about what languages the injured worker can speak, educational background including vocational training and education beyond high school, and whether the injured worker is involved in any organizations, clubs, charities or associations of any kind, either as a volunteer or member.

In addition, there is a warning at the top of the form that now states in bold “If permanent total disability is granted, the injured worker is not permitted to return to work in any capacity.”

Although the prior form will not be rejected by the Industrial Commission, the new form has information that will likely be requested by hearing officers at hearings on whether or not the injured worker has reached permanent total disability.
